Student Applications Due to State Chairman: January 1​
The following documents must be attached to the application when submitted, single side pages only:

1. Financial Aid Form

2. Complete official academic transcripts

3. Letter from applicant, limit of one page only

4. List of extracurricular activities, honors, recognitions received within the last three years

5. Three letters of recommendation

6. All application documents must be attached to the online application

7. Applications must be submitted online.  Please note Student Applications are due to State Chairman January 1

8. When a student reapplies for a scholarship (whether the applicant is or is not a previous winner), a completely new and updated application must be submitted.
